Welcome to the COUPLE QUESTIONNAIRE!

This Questionnaire will help me personalise your ceremony and get to know you two better. I know that this task will fall on very different ends of the Fun/Daunting Spectrum for people, and that your entire relationship can’t really be boiled down to just a few generic questions—that said, just fill this out in the way that best suits you, and I’ll weave your responses together into a story that captures your relationship in the way that it deserves to be captured.

A few things:

• This Questionnaire should be filled out individually (not as a couple!) at least two weeks prior to your wedding date

• Your answers can be as short n sweet or as long and detailed as you want them to be

• Leave anything that you don’t want to answer blank, no questions asked

• Anything shared in your answers will be taken as permission for me to use it during your ceremony (unless you specifically say otherwise, eg. sharing it for context only)

• If you hate sitting at a computer and writing out your feelings—No probs! Just let me know, and we can organise a phone call interview instead

• If you’d like to really give consideration to your responses, I recommend that you copy and paste the questions into a Word Doc/email draft/Note and work away at your answers over a couple of weeks or months, then send them through to me once you’re ready
